- I undertake to notify the local Divisional Veterinary Manager immediately if there is a significant change of management or staff or the licensed premises undergoes significant structural repair, maintenance or refurbishment;

Notes
- A ""dedicated slaughter collection"" means an animal gathering for the purpose of onward consignment direct to slaughter in Great Britain.
- An animal area is an area to which animals may be given access.
- The Order means the Foot-and-Mouth Disease (Scotland) Order 2006.
- Once completed this application must be submitted to the DVM at your local Animal Health office.
Schedule of Conditions
Condition relating to the premises
1. The site must be of impermeable material capable of being cleansed and disinfected fully.
2. It must have secure boundaries to prevent escape of livestock and their waste products.
3. The collection centre must not be adjacent to any field, land or premises that contains or may contain susceptible livestock.
4. Floors, fixture and fittings should be in a good state of repair and capable of being cleaned and disinfected.
Conditions relating to the operation of the premises
1. The site is only to be used for the purpose of a collection centre within the boundaries of the premises. No gathering of non-susceptible animals or other livestock activities and no gathering of people not connected with the licensed activity may take place on the premises at any time it is operating as a licensed premises under the Order.
2. The Local Authority and the Divisional Manager should be notified if a collection is to be held, prior to the animal gathering.
3. Records of all staff who have worked at the collection centre, including temporary staff must be made and retained for 6 months to enable tracings if required.
4. Every animal shall be inspected by a Local Veterinary Inspector (LVI) for clinical signs of foot-and-mouth disease and in any event prior to onward transport.
5. No animal may enter the premises before midnight on the day before a gathering and all stock must leave the premises by midnight on the day of the animal gathering. There must be contingencies agreed with the DVM to deal with remaining animals if eg. an abattoir breaks down, and must include a policy for the culling of animals not fit for human consumption or transport.
6. The licensee must ensure that the number of animals allowed onto the premises for any gathering is controlled so that all of these conditions can be met.
7. The licensee must appoint a biosecurity officer, responsible for ensuring compliance with requirements of the Disease Control (Interim Measures) (Scotland) Order 2002.
8. BCMS & SAMU must be electronically notify of movements within 24 hours of collection to assist with tracings should that be required.
9. There must be personal washing and disinfecting facilities on entering and leaving premises.
10. Entry into the livestock area of the collection centre should be by essential staff only unless authorised by the biosecurity officer or if they are an inspector or a person accompanying an Inspector. Any person entering the animal area during a gathering (including livestock vehicle drivers) must:
(a) wear outer clothing that can be cleansed and disinfected and which is cleansed and disinfected immediately each time the person leaves the animal area, or
(b) wear protective clothing that is removed immediately each time the person leaves the animal area, has no contact with a susceptible animal and is laundered before it is used again, or
(c) wear disposable protective clothing which must be disposed of immediately each time the person leaves the animal area such it has no contact with a susceptible animal.
(d) wash his hands each time the person leaves the animal area; and
(e) wear boots that can be cleansed and disinfected and are cleansed and disinfected each time the person leaves the animal area (and before entering any vehicle).
11. The facilities for cleansing and disinfection on the premises must themselves be cleansed and disinfected after the last livestock vehicle has left the premises following a gathering and before any other livestock vehicle enters the premises.
12. There must be waste management protocols for disposal of manure and bedding. It will be a requirement that all feedingstuffs to which animals had access to, and all bedding, excreta, other material of animal origin manure is held until further notice, so that animals do not have access to them.

1. Cleansing and disinfection must be with a disinfectant approved for use against FMD under the Diseases of Animals (Approved Disinfectants) Order 1978. 2. To avoid backing up in the loading area and to ensure throughput of all livestock in one day, there must be careful consignment planning by the collection centre operator. The operator must ensure they have the logistics in place to ensure that this can be effectively achieved.
3. The Licensee should ensure all vehicles are cleansed and disinfected before departure (Local Authorities will supervise as appropriate).
4. Animal Health and/or Local Authority may be in attendance where they see fit. This will be decided on a risk basis.
5. Multiple drop offs to different abattoirs are not permitted.
6. Failure to observe the measures required in this licence is an offence under Section 73 of the Animal Health Act 1981 and may result in this licence being withdraw. The penalty on conviction of an offence under this section is a maximum of six months imprisonment and/or a £5,000 fine.
